2021 Annual Report: 15 Stories for 15 Years
 
Patient health plays an important role in the Genesee County Office for the Aging’s work. The agency provides information, support and advocacy for residents ages 60 and older to promote independence and improve quality of life.

Caseworkers receive alerts about their clients’ hospitalizations, emergency department visits and discharges through RHIO, making it possible for them to follow up with timely and appropriate care.
 
“RHIO access allows us to reach out to our clients in nearly real-time after a discharge or other event,” says Barbara Hendershott, supervisor. “It helps our caseworkers develop appropriate plans and informs them of any new issues that may warrant additional services.” 
 
When it comes to serving their patients, Hendershott says RHIO helps caseworkers better serve their clients and keep them safe.
 
“Recently a RHIO alert indicated that a client was in a very serious health situation, prompting us to reach out,” says Hendershott. “That helped us get the information we needed to implement a timely, customized care plan once the person was released.”
 
Alerts support the critical work that caseworkers perform at the point of care.
